2012 W-2 forms are available online now. Paper W-2 forms WILL BE MAILED January 18, 2013.
You can access your W-2 form (Annual Wage and Tax Statement) online by simply consenting to an online W-2 form. This is done through the Employee menu on CCAC Central. There is a CCAC Central button in the lower left corner of every CCAC webpage (including this one - see below).
Whether you consent or not the college will mail a paper W-2 form to all employees again for tax year 2012. They will be mailed by January 31, each calendar year.
Again, to get an online W-2 form you must first give your consent. These step-by-step instructions show you how to consent to get an online W-2 form.
Local Wage Tax Withholding begins in 2012
Act 32 requires all employees to have their local wage taxes withheld from their paycheck. CCAC is required to remit these taxes monthly to the appropriate tax collection district.
In order for your municipality to get the taxes CCAC will withhold from your pay, we must obtain certain information for reporting purposes. You are required by Act 32 to supply your employer with this information. You may submit a paper Local Earned Income Tax Residency Certification Form or go online through the Employee menu on CCAC Central (see the button below) to submit this information electronically. New employees must submit a paper form which is included in the New Hire Packet.
Also, read the INSTRUCTIONS on the Act 32 webpage as these instructions contain information you will need to complete the Employer Information –Employment Location section.
On this form you must supply the Resident PSD code for the municipality in which you live. If you are cannot determine this code by using the website www.newPA.com, then you should request this code from the wage tax office to which you remitted local income taxes prior to 2012.
Submit the paper Local Earned Income Tax Residency Certification Form to the following address: CCAC-Payroll Department, 800 Allegheny Ave, Pittsburgh, PA 15233
Follow these step-by-step instructions to submit an online Certificate of Residency form.
Thank you in advance for submitting this information in a timely manner.
Local Services Tax Reminder
If CCAC is your secondary employer you must submit at the beginning of each calendar year a Local Services Tax Exemption form along with proof that you are having the Local Services Tax deducted by your primary employer.
Stop Payments on Lost/Stolen Paychecks
You will be required to complete a Stop Payment Request Form when you ask for a stop payment to be placed on your paycheck. The College has begun to charge employees a Stop Payment fee of $20.00 which will be deducted from the replacement check to recover the costs involved with replacing the paycheck.
The Payroll web pages contain various references to documents. When you click on the link for a particular document you will be directed to the Payroll Department Forms Web page where you will need to select the form you want from an alphabetical list of documents. This has been done to facilitate updating these web pages; to insure that you retrieve the most current version of the document you desire.
